Overzicht
Tirzepatide is een synthetisch peptide dat twee verwante darmhormonen tegelijk nabootst: glucose-dependent insulinotropic polypeptide (GIP) en glucagon-like peptide-1 (GLP-1). Waar eerdere middelen op één incretine-receptor aangrijpen, activeert tirzepatide er twee: het is een dubbele agonist.
De basis is een 39 aminozuren lang peptide rond de GIP-sequentie, met Aib op posities 2 en 13 tegen afbraak en een C20-di-carbonzuur op een lysine dat aan albumine bindt; de halfwaardetijd is ongeveer vijf dagen (wekelijkse toediening).
Tirzepatide is het compound achter geregistreerde geneesmiddelen (o.a. Mounjaro, Zepbound). Die regulatoire status is een feit over de betreffende toezichthouders. Binnen Peptalis uitsluitend research use only: in-vitro, niet voor humaan of diergebruik.
Werkingsmechanisme
Tirzepatide activeert zowel de GLP-1- als de GIP-receptor. Het is een imbalanced agonist met een profiel dat sterker naar de GIP-receptor neigt.
Dubbele incretine-activatie
- Via de GLP-1-route versterkt tirzepatide de glucose-afhankelijke insuline-afgifte, remt het glucagon en dempt het de eetlust.
- Daarbovenop komt de GIP-route, die eveneens de insuline-respons en het energiemetabolisme beïnvloedt.
Imbalanced / biased signalering
- Receptor-farmacologisch werk beschrijft tirzepatide als een imbalanced agonist die op de GLP-1-receptor een enigszins vertekende signalering vertoont (Willard 2020).
- Hoe die onbalans precies bijdraagt aan de klinische uitkomsten is een open vraag.

Preklinisch bewijs
- In het ontwikkelwerk werd tirzepatide (LY3298176) geïdentificeerd als één molecuul dat beide incretine-receptoren activeert en in diermodellen glucosecontrole en lichaamsgewicht gunstig beïnvloedt (Coskun 2018).
- Receptor-farmacologisch werk karakteriseerde het imbalanced/biased signaleringsprofiel (Willard 2020).
Humane studies
- SURPASS-2 (Frías 2021): 1879 mensen met type 2-diabetes; tirzepatide 5/10/15 mg per week direct vergeleken met semaglutide 1 mg; HbA1c-daling −2,01 tot −2,30 vs −1,86 procentpunt, met grotere gewichtsdaling in de tirzepatide-groepen.
- SURMOUNT-1 (Jastreboff 2022): 2539 volwassenen met obesitas zonder diabetes; bij 15 mg per week gemiddelde gewichtsverandering −20,9% vs −3,1% placebo over 72 weken.
